Health Services
The D155 Health Services Department supports student health, safety, and wellness so students are ready to learn each day. Each school has a qualified nurse on duty during all student attendance days to provide care, respond to health concerns, and support student well-being.
Health Services provides first aid and emergency care, assesses and responds to illness and injury, administers medications, and offers health counseling. School nurses also work with families to help students safely manage chronic health conditions such as diabetes, seizures, severe allergies, and asthma during the school day.
In addition, Health Services manages required health records, physical and immunization compliance, and vision and hearing screenings in accordance with the Illinois School Health Code. The department also partners with local health agencies to monitor and respond to communicable disease concerns.
Physical Examination and Immunization Requirements
9th Grade:
- Complete 9th Grade Physical Examination and all required immunizations - must be turned in prior to October 15th
- Complete 9th Grade Dental Examination - must be turned in prior to May 15th
10th Grade:
- No additional physical/immunization requirements if up to date.
11th Grade:
- No additional physical/immunization requirements if up to date.
12th Grade:
- Must show proof of receiving two doses of meningococcal vaccine (meningitis) with the second dose received on or after the 16th birthday. If the first dose was given after the age of 16, then only one dose is required.
Freshman Requirements
To ensure health and safety for all, students entering 9th grade are required to have a current physical examination and immunizations on file in the school health office before the first day of school and preferably by August 1. Students who do not submit required health documents by the first day of school may be excluded from school until documentation has been received per Board Policy 7:100.
The physical examination needs to be dated within the 12 months prior to freshman distribution day. Your physician will need to sign this form in two places: 1. The immunization portion; and 2. The physical examination portion. A Parent/Guardian needs to complete, sign, and date the “Health History” section on the back of the physical form prior to the appointment with a health care provider. If your student is interested in participating in athletics, an additional copy of the Certificate of Child Health Examination must be submitted to the Athletic Department.
Senior Requirements
All students entering 12th grade must show proof of receiving two doses of Meningococcal vaccine unless the first dose was administered after the age of 16. In this case, only one dose of Meningococcal vaccine after the age of 16 is required.
Students new to Illinois will have 30 days from the enrollment date to submit documentation of the required immunizations and physical exam, and must also complete a vision exam.
Religious objections to any physical examination or immunization must be submitted by a parent/guardian in writing and must contain the required information.

All students entering 9th grade are required to have a dental examination. Dental examinations are due by May 15 of the year the student is enrolled in 9th grade. A current examination is one that was completed within 18 months of May 15 of the school year.

All students enrolling for the first time in a public school shall submit proof of an eye examination completed within the previous year before October 15 of the school year.

Students with asthma are required by Illinois law to have an Asthma Action Plan on file with the school nurse. The Asthma Action Plan is a written plan developed with the student’s medical provider to help manage and control asthma symptoms.
If a student will self-carry or self-administer asthma medication, parents or guardians must provide written authorization to the school nurse. This authorization must be renewed each school year. The Asthma Action Plan must be maintained in the school nurse’s office.

Physician Authorization of Treatment is to be completed by a physician and signed by a parent/guardian for students who require specialized nursing care during the school day.

Students with diabetes are required to have a Diabetes Medical Management Plan (DMMP) on file. A new plan must be provided to the school every school year and updated as needed.

The State of Illinois passed a Public Act in 2006 that permits students to carry and self-administer epinephrine auto-injectors with the written authorization of a physician and parent/guardian. Authorization needs to be renewed each school year. A completed Food Allergy & Anaphylaxis Emergency Care Plan should be on file in the nurse’s office for students with severe allergies or who are at risk for anaphylaxis.

If your student requires medication at school, the medication authorization form must be on file in the nurse’s office. You may download this form using the steps listed above. This form, signed by both a physician and a parent/guardian, is required for both prescription and over-the-counter medications, including Tylenol, aspirin, Midol, antacids, etc., as well as asthma inhalers, seizure medications, and Epi-pens. Students may carry inhalers, seizure medication, and Epi-pens with them during the school day per their Asthma Action Plan and/or their Seizure Action Plan on file with the school nurse. Medical Cannabis administration requires a separate form specific to this medication. A new form for all medications must be completed every school year.

Emergency (Non-Student Specific) Medications
In accordance with Illinois School Code, Community High School District 155 maintains a supply of undesignated (non-student specific) emergency medications in each school. These medications may be administered by a trained school nurse or other trained personnel to any student or individual experiencing a life-threatening emergency while on school grounds or at a school-sponsored activity.
Emergency medications maintained by the District include:
- Albuterol rescue inhalers for respiratory distress related to asthma
- Epinephrine auto-injectors (EpiPens) for severe allergic reactions (anaphylaxis)
- Glucagon (including Baqsimi) for severe hypoglycemia (low blood sugar)
- Naloxone (Narcan) for suspected opioid overdose
These medications are not intended to replace medications prescribed to individual students. Families are encouraged to continue providing required student-specific medications and health documentation to the school nurse.
