Fall 2008 Registration
Every student or parent must attend on-site
registration at the designated time (listed below) in order to receive books, student ID, class schedule, and to turn in
certain forms (including physical forms, emergency "pink" cards, and free & reduced lunch waivers).

Registration Fees
The standard registration fee for all students is $66. This fee covers the rental cost for all of your child's text books, and reflects a considerable savings over the purchase of these books. Depending on the individual student's schedule other costs for items such as course-specific workbooks must be paid for during registration as well.
During on-site registration you may pay using cash, check, Visa, or MasterCard.
Freshmen must bring the following:
- Current physical examination form, signed by a physician and dated within one year of registration.
- Proof of up-to-date immunzation recorded on the school health form and signed by the healthcare provider. All students must be properly immunized against diphtheria, pertussis, tetanus, polio, rubella, and mumps.
- Updated emergency information card - this pink card was mailed to all students, and must be completed and signed by a parent or guardian.
- Illinois Dept. of Human Services Physical Form in PDF format
- Medication Authorization Form in PDF format
- IHSA Athletic Physical Form in PDF format
Note: The IHSA Athletic Physical Form is necessary to compete in all sports, but is not accepted for freshman registration purposes. All freshmen must have a completed Illinois Department of Human Services Physical Form for registration.
Behind the Wheel Driver Education Fee = $65.00
State of Illinois Fee = $20.00
Physical Education Uniform
All freshmen must purchase a Prairie Ridge P.E. uniform consisting of a t-shirt and shorts. Uniforms will be available for purchase during on-site registration and online pre-registration. The total cost for a uniform is $15.00.
Yearbook & Student Identification Card Pictures
Yearbook photographs will be taken during registration for freshmen, sophomores, and juniors. Please wear appropriate clothing. Additionally, pictures for freshmen student ID cards also will be taken at that time.
Transportation & Bus Information
A representative from the Transportation Office will be at registration to answer your bus related questions.
