H1N1 FLU VACCINATION CLINICS

Clinics available for D155 students February 1-4

The H1N1 (swine) flu vaccine is available to the public through public clinics, pharmacies, and primary care physicians. In partnership with the McHenry County Department of Health, Centegra is administering vaccination clinics at schools throughout McHenry County. Centegra will host a total of four clinics to serve District 155 students, with two of these clinics taking place at District 155 schools and two at District 47 schools (see chart below). Note that these clinics are for District 155 students and those students from District 3, 26, 46, and 47 only, and are not open to the public.

The full H1N1 information packet and consent form is available for download here in PDF format in English and Spanish. The consent form must be returned to your child's school nurse by Thursday, January 28th. Vaccinations will not be administered without a consent form. All of the information in each section of the consent form must be filled out completely (including the gray area in the middle of the form).

• The vaccine will be administered in shot form only. The nasal spray will not be available.
• The CDC has eliminated any priority grouping for receiving the vaccine. All District 155 students are eligible for the vaccine if desired and if parents submit the consent form as instructed.
• Centegra is requesting a $15 fee to pay for the cost of administering the vaccine. Cash or a check payable to Centegra should be submitted at the time of the clinic. No student with a signed consent form and an Illinois All Kids card will be turned away.
• Centegra—not school officials—will be responsible for administering the vaccination and clinic operations for each building.
• Students are advised to wear attire with the ability to easily roll up the sleeve.
