EMERGENCY CARD (PINK):

All parents must complete the enclosed pink “emergency card” and bring the card to registration. This card is vitally important to your child’s welfare in the event of an illness/accident occurring at school.  You may download this card at www.d155.org/pr , Forms link.

 

  • Cards are renewed annually.
  • List all phone numbers including cell phone and Parents e-mail address.
  • Fill out both sides of the card.
  • Parent/guardian signature is required.

 

STUDENTS WILL NOT BE ALLOWED TO REGISTER WITHOUT A PINK CARD.

FRESHMAN PHYSICALS AND IMMUNIZATIONS:

The Illinois Department of Human Services requires that all 9th grade students must have a physical examination by a licensed physician or nurse practitioner within one year prior to entry into high school. The physical and immunizations must be documented on the revised Illinois Department of Human Services form which was enclosed in the 8th grade packet distributed in January.  Physicals recorded on the IHSA Athletic Physical form are not accepted by the Illinois Department of Human Services for entry into high school.  You may download this DHS physical form at www.d155.org/pr, Forms link.

The student must also have received the minimum immunization requirements.  The Illinois state law requires a Td booster every ten years as a minimum immunization requirement.  Please submit written proof of a current Td booster to the school nurse at registration in August.

 

INCOMING FRESHMAN WILL NOT BE ALLOWED TO REGISTER WITHOUT

A CURRENT PHYSICAL,

COMPLIANT IMMUNIZATIONS,

AND PINK EMERGENCY CARD

 

MEDICATION FORMS:

If your son/daughter requires medication at school, the enclosed medication authorization form must be on file in the Nurse’s Office. Please read the procedure for administration of medication at school on the back of the form.  Forms are renewed annually.  You may download this medication form at any time at www.D155.org/pr, Forms link.

 

  • Parent and physician signatures are required.
  • Form is required for prescription and/or over the counter medications
  • (THIS INCLUDES TYLENOL, ASPIRIN, MIDOL, ANTACIDS, ETC...)
  • Use this form for asthma inhalers and Epi-pens. 
  • Students may carry inhalers and Epi-pens with them during school.
